What information we collect and why

 We collect and use personal information to deliver personalised coaching services, schedule and manage appointments, process payments, and issue invoices.

 The personal information we may collect includes:

  •  Name, address, and contact details

  • Gender

  • Date of birth

  • Next of kin details

  • Health information (including medical conditions, allergies, medical requirements, and medical history)

  • Information about care needs (including disabilities, home conditions, dietary requirements, and general care provisions)

  • Test results (including psychological evaluations, scans, bloods, x-rays, tissue tests, and genetic tests)

  • Payment details (including card or bank information for transfers and direct debits)

 Special category data

 Some of the information we collect is considered special category data because of its sensitivity — for example, health information. This type of data receives additional protection under the law.

 Lawful bases for using your information

 Under the UK GDPR, we must have a lawful basis to collect and use your personal information. Depending on the circumstances, we may rely on the following bases:

 Where we get your information

 We usually collect your personal information directly from you.

 How long we keep your information

 We keep your personal information for six years, in accordance with UK legislation, unless the law requires us to keep it longer.

 Who we share your information with

 We only share your personal information when necessary, and only with:

  • Other health providers (e.g., GPs and consultants), with your consent only.

 Duty of confidentiality

 We are bound by a common law duty of confidentiality, meaning your information will only be shared where:

  • You have provided consent (explicitly or impliedly for care purposes);

  • We have a legal obligation (such as a court order);

  • There is an overriding public interest (for example, to prevent or detect serious crime);

  • We are required to share under The Health Service (Control of Patient Information) Regulations 2002 (England and Wales); or

  • We have authority from an appropriate body in Scotland (such as the Chief Medical Officer or Public Benefit and Privacy Panel for Health and Social Care).

 Your data protection rights

 Under UK data protection law, you have the following rights:

  • Right of access – to ask for copies of your personal data.

  • Right to rectification – to correct inaccurate or incomplete data.

  • Right to erasure – to request deletion of your data.

  • Right to restrict processing – to ask us to limit how your data is used.

  • Right to object – to object to certain types of processing.

  • Right to data portability – to request that your data be transferred to another organisation or to you.

  • Right to withdraw consent – where we rely on consent, you can withdraw it at any time.
